Before your child’s first day
We need these four back, filled out and signed, before your child can start.
Identification and Emergency Information

Gives us permission to seek medical care for your child right away if we cannot reach you first.
Parent Questionnaire
Tells us about your child’s routines, comfort items, allergies, and the little things that help them feel settled with us.
Parent Notification (Parents’ Rights)
Your rights as a parent of a child in licensed care. Read it, sign it, and keep a copy for yourself.

Infant Sleep Plan
For babies under one year. Tells us how your little one sleeps so we can follow safe sleep practices and your routine together.
Photo Release
Optional. Lets us know whether your child can appear in photos we share with families or post online.
